Patient representative
Simply stated, our Patient Representative is here to represent the patient. Illinois Valley Community Hospital is devoted to providing patient-centered care. Our ultimate goal is to provide you with the best healthcare and medical service possible. Many times we exceed expectations. Every now and then, however, we may fail to meet our patients’ or families’ expectations. We want to learn from your experience, good or not so good.
The Patient Representative has the authorization to extend your compliments or to look into your complaint. If you have a concern, we will work to resolve your issue in a neutral manner with all involved parties.
You may contact the Patient Representative, Deb Puetz RN, during your hospitalization by calling extension 3592 Monday through Friday from 7:30 a.m. to 4:00 p.m. The Patient Representative’s direct telephone line from outside the hospital is 815-780-3592.
You may also visit the Patient Representative office located on the 3rd floor (3R elevator), just off the Skywalk in the Quality Management department adjacent to Medical Records department. We are here to enhance your hospitalization experience by ensuring your voice does not go unheard.
