IVCH Diabetes Center for Excellence

Take control of your diabetes, ask your doctor for a referral!

The Illinois Valley Diabetes Center for Excellence offers first-class services for those dealing with pre-diabetes, gestational diabetes, Type 1 and Type 2 diabetes. The Center is accredited by the American Association of Diabetes Educators, and is taught by certified diabetes educators, pharmacists, registered dietitians, physical therapists, licensed social workers, and nurse educators.

Diabetes Education

If you or someone you know has diabetes, Illinois Valley Community Hospital offers a comprehensive program that includes diabetes self-management classes in addition to individual appointments with a dietitian and a diabetes nurse educator. Diabetes self-management education includes gaining the knowledge, skills and confidence necessary for taking control of your diabetes. Our mission is to empower people of all ages to appropriately self-manage their diabetes so they may benefit from lifelong good health. Our team of experts in diabetes care offers a variety of services and will work with your entire health care team to improve your health status and quality of life.

Individual Diabetes Education

One-on-one diabetes education is offered for pre-diabetes, gestational diabetes, type 1 diabetes, type 2 diabetes, medication administration and instruction, blood glucose monitoring, insulin pump therapy and all other diabetes-related care. A physician   order is required. The education includes appointments with the nurse educator   and/or registered dietitian and is held on the main campus of IVCH. Costs may be covered by insurance. Diabetes Education Classes

Group education classes are offered as a four-part series. You and your physician can decide if all four classes are needed, or if certain classes would be best for you. A physician referral is required to attend classes. Costs may be covered by insurance. Guests are welcome to attend classes with you. Diabetes self-management classes will take place at the IVCH 6th street building—1305 6th Street, Peru, IL.

  • Class #1: Diabetes overview and goal setting
  • Class #2: Healthy eating and being active
  • Class #3: Diabetes medications, blood glucose monitoring, problem solving for acute complications
  • Class #4: Healthy coping, stress, reducing risk of chonic complications

June June 3, 10, 17 and 24 at 6 to 8 p.m.
July July 1, 8, 15 and 22 at 9 to 11 a.m.
August August 5, 12, 19 and 26 at 6 to 8 p.m.
September September 2, 9, 16 and 23 at 9 to 11 a.m.
October October 7, 14, 21 and 28 at 6 to 8 p.m.
November November 4, 11, 18 and 25 at 9 to 11 a.m.

Diabetes Academy

Meet with your peers and diabetes educators to support one another and gain new perspectives in diabetes self-management. This support group meets monthly, and a different diabetes-related topic is discussed each month. Guest speakers and/or IVCH Diabetes Center for Excellence staff conduct the presentations. There is no fee to attend the group. All meetings are held at the IVCH 6th Street Office Building, 1305 6th Street, Peru, IL. Find information on our next Diabetes Academy meeting by calling 815.780.3576.

From West Street, turn east onto 6th Street. Drive 1 ½ blocks east, building will be on your left. Parking is available in the lot behind the building or on 6th Street (street parking).
