Job Description

POSITION SUMMARY

The Social Worker is responsible for assessing patients and providing discharge planning, education and counseling for all patients referred, as well as coordinating the care transitions with all disciplines within the hospital and community resources outside of the hospital.

Qualifications

Education / Experience:

• Bachelor's or preferably Master's Degree in Social Work required or
• BSN in nursing required
• at least 2 years’ experience in health care

Licensure / Certification:

• LSW or LCSW; Current license through the State of Illinois or
• Current RN licensed in Illinois

• Electronic documentation
• Microsoft knowledge
