Patient Representative

Let Us Know What You Think about Your Experience with Us

Simply stated, our Patient Representative is here to represent the patient. Illinois Valley Community Hospital is devoted to providing patient-centered care. Our ultimate goal is to provide you with the best health care and medical service possible. Many times we exceed expectations. Every now and then, however, we may fail to meet our patients' or their families' expectations. We want to learn from your experience, good or not so good.

The Patient Representative has the authorization to extend your compliments or to look into your complaint. If you have a concern, we will work to resolve your issue in a neutral manner with all involved parties.

How to Contact Your Representative

You may contact Patient Representative Deb Puetz, RN during your hospitalization by calling extension 3592 Monday through Friday from 7:30 a.m. to 4:00 p.m. The Patient Representative's direct telephone line from outside the hospital is 815.780.3592.

You may also visit the Patient Representative office located on the third floor (3R elevator), near the Skywalk in the Quality Management department, across from the Medical Records department. We are here to enhance your hospital experience by ensuring your voice does not go unheard.

Your comments are very important to us as we strive to increase patient satisfaction at IVCH. If a concern you have shared has not been addressed to your satisfaction, you may contact:
