Job Description

Responsible for easing the transition from an institutional setting to the home care setting. Follow up on home care needs with patient assessments and provide education on disease processes.


Education/Experience: Graduate of an approved school of nursing; Completion of an associates degree level R.N. or higher, BSN preferred; Minimum five years' clinical experience required; Home health experience preferred

Licensure/Certification: Current unencumbered Illinois Registered Professional Nurse license; Valid Illinois driver's license

Other Knowledge/Skills: Excellent written, verbal, and listening skills; Willingness to establish relationship with internal and external customers; Ablity to manage conflict, stress, and multiple simultaneous work demands in a professional, effective manner; Ability to work independently, while collaborating with other team members; Ability and willingness to self-motivate, prioritize, and be willing to change processes to improve effectiveness and efficiencies; Adapts to changing patient or organizational priorities; Ability to make independent decisions in accordance with established policies and procedures; Decisions and problem solving require a combination of analysis, evaluation, and interprtive thinking; Computer literacy, including but not limited to data entry, retrieval, report generating
